Services

Home and Hospital Teachers provide instructional services for students who are physically unable to attend school and must remain at home or in a hospital for medical reasons and for a period of 14 to 60 days in any school year.

Home and Hospital Instructional Services

Home and Hospital Instructional Services are requested by a physician. The student's physician must complete a Physician's Affirmation of Need for Temporary Home or Hospital Education for Medically Necessary Reasons in its entirety and submit it to the students' school building principal.

Process

Caregivers must submit the completed Physician's Affirmation of Need for Temporary Home or Hospital Education for Medically Necessary Reasons to the student's school principal. The principal or their designee will electronically submit a Referral Form to the Office of Home and Hospital Instructional Services. Within 5 school days of receiving the referral, a HHIS teacher will be assigned to the student. The teacher will contact the caregiver to schedule services and share the student’s instructional learning plan. Home and Hospital Instructional Services will be provided for up to 60 days. A return to school meeting will be held to communicate medical and academic information so the student has a successful return to school.
